Third and fourth players from same family (household) are half price, ranked by age. After 2/20/2010 a $25 late fee will be charged.

Consent: I do hereby give my consent for the above named player to participate in the Clifton Park Baseball League. I will assume all risks and hazards that are incidental to the activities. I further agree to release, absolve, indemnify and hold harmless this baseball league, their sponsors, organizers, coaches and supervisors of all legal responsibility. I give my permission to this league, its officers or representatives to provide medical treatment to the above named child in case of an emergency or injury. In the event of an emergency, I also give my consent that treatment or any medical staff member on duty can provide tests.
